- Medical doctor (MD)
Use the Medical Board's search for an MD, a medical doctor. It lists licence details and the board's public decisions.
MD: search the Medical Board- Doctor with a DO licence
DO is a doctor licence handled by a different Arizona board from the one for MD doctors. Use that board's Find your DO search.
DO: use the separate search- Nurses, including nurse practitioners
Extra certification allows a nurse practitioner, a type of nurse, to assess health problems and make care decisions. Prescribing medicines also needs the Nursing Board's permission. The board links to Nursys, a licence-check system, to check that certificate and the nursing licence.
Check the nurse's record- Physician assistant (PA)
A physician assistant, called a PA, can prescribe within their training and skills. Depending on experience approved by the board, they may need a signed agreement for a doctor to supervise their work. PAs have their own Arizona board for the licence check.
Search a PA licence- Other doctors with a limited drug licence
These doctors have a separate licence from MD and DO doctors. They may prescribe some medicines, but some cancer treatments and medicines for serious mental illness are excluded. Drugs with misuse controls and prescription drugs given through a vein also have limits. Their state licensing office is the Naturopathic Physicians Medical Board; ask it about the exact medicine and the person's permission to prescribe.

A nurse practitioner's permission to prescribe is limited to the patient group covered by their training.
Check the pharmacy as well.
Arizona pharmacy rules as of October 3, 2026.
A pharmacist's licence and the pharmacy's permit are different Arizona records.
A pharmacy based outside Arizona needs a permit from this state to send medicine here, plus authority in its home state.
The Food and Drug Administration (FDA) is the US drug agency; its links take you to state pharmacy licence searches.
FDA: locate a state-licensed pharmacyIf a lookup is hard to use, note the pharmacy's exact name and request permit confirmation from the Arizona State Board of Pharmacy.
A blank history leaves questions open.
Arizona's rules on October 6, 2026 restrict the board records shown online; an empty listing leaves open whether any complaint or action occurred.
You can ask the board for additional public records about the licence. Pending complaints and investigations have disclosure limits, so not every complaint will be available.
Records of public meetings, called minutes, and meeting notices are outside the law's usual limits on website records, including its five-year limit.
Give the board the person's name and licence number. Ask for public records of past complaints or board actions, including older decisions and meeting records about that person.

As of October 3, 2026, many private offices of Arizona-licensed doctors and other health professionals do not need a separate health-facility licence, a state licence for the care site.
The exemption excludes offices with overnight stays, freestanding urgent-care centres, and abortion and pain-management clinics defined by law. General anesthesia, medicine that makes a patient unconscious, is another exception, with a special rule for dental care.
Checking the care site's licence cannot replace checking the person who prescribes your medicine.
Ask the Health Department whether the exact services at the office require a facility licence. Give it the office's name and address; a missing search result alone is not enough to decide.
